This Privacy Policy (“Policy”) sets out the bases which Calibrate Health Pte Ltd may collect, use, disclose, process and protect your personal information. In this Policy, “we”, “us”, or “our”, or “Calibrate” collectively refers to Calibrate Health Pte Ltd and partner affiliates.

As used in this Policy:

  • patient” and “customer” means an individual who (a) has contacted us through any means to find out more about any goods or services we provide, or (b) may, or has, entered into a contract with us for the supply of any goods or services by us; and
  • personal information” means information or data about a customer who can be identified: (a) from that information; or (b) from that information to which we have or are likely to have access.

This Policy applies to personal information collected by Calibrate in connection with the services and products we offer. This includes information collected offline at Calibrate clinics, direct marketing campaigns, contests, competitions and online through our apps and websites. This Policy also applies to targeted Calibrate content, including offers and advertisements for Calibrate services and products which we may send to you on third-party websites, platforms and applications. These third-party sites may have their own privacy policies and terms and conditions.

Information We Collect

Depending on the nature of your interaction with us, some examples of personal information which we may collect from you include:

  • name, residential address, email address, telephone number, gender, date of birth, and employment information;
  • credit card and other payment information;
  • medical history;
  • insurance information and GP practice information;
  • information, feedback or content you provide through marketing campaigns, sweepstakes, contests, surveys, or promotional offers

We generally do not collect your personal information unless (a) it is provided to us voluntarily by you directly or via a third party who has been duly authorised by you to disclose your personal information to us, or (b) collection and use of personal information without consent is permitted or required by the Personal Data Protection Act (PDPA) or other laws. When you agree to this Policy you are, to the extent required under your local law, granting your express and written consent to the processing of any personal information that you provide to Calibrate.

Information We Collect When You Visit Us Online

When you visit out websites, we use a web analytics tool to analyse site usage, how you arrive at our site, what time and date you visit our website, what pages you visit, the time spent on each page, what you do on our website, what browser you are using and on what internet browser you are using etc. This information is not tied personally identifiable information.

Cookies
We use cookie technology on our websites to allow us to evaluate and improve their functionality. Cookies by themselves cannot be used to reveal your identity. They identify your browser, but not you, to our servers when you visit our websites and apps. Targeted Advertising
Where permitted by law, we may work with other companies to show you advertisements we think you may find relevant and useful. The advertisements you see may be based on information collected by us or third parties and/or may be based on your activities on our websites or third-party websites.

External Web Services

We may use external web services to display content on our website including but not limited to YouTube (to show videos). We cannot prevent these sites or external domains, from collecting information on your usage of this embedded content.

Security

Because the security of your personal information is important to us, we use Secured Socket Layer (SSL) software in order to encrypt the personal information that you provide to us. If your browser is SSL-enabled (which most are), your transmission of personal information to us online will be encrypted.

We store your collected personal information (online and offline) on databases protected through a combination of physical and electronic access controls, and other reasonable security measures. Nevertheless, such security measures cannot prevent all loss, misuse or alteration of personal information and we are not responsible for any damages or liabilities relating to any such incidents to the fullest extent permitted by law. How We Use Your Personal Information

We use your personal information to provide you with a smooth, efficient, and personalised experience while using our services. This allows us to provide services and products that meet your needs, and to customise our services to make your experience more efficient and easier.

We reserve the right to disclose any personal information about you to law enforcement or other government officials as we, in our sole discretion, believe necessary or appropriate, in connection with an investigation of fraud, Intellectual Property Rights infringements, if we are compelled to do so by court of law or lawfully requested to do so by a governmental entity or if we determine it is necessary or desirable to comply with the law or to protect or defend our rights or property in accordance with applicable laws.

Transfer Of Your Personal Information

We generally do not transfer your personal information to countries outside of Singapore. However, if we do so (for example sending you case notes to an Osteopath or Physiotherapist outside Singapore if you move out of Singapore) we will take steps to ensure that your personal information continues to receive a standard of protection. By submitting your personal information, you agree and consent to us transferring your personal information if needed, in accordance with the terms of this Policy and applicable data protection laws and regulations.

Protection Of Your Personal Information

To safeguard your personal information from unauthorised access, collection, use, disclosure, copying, modification, disposal or similar risks, we have introduced appropriate administrative, physical and technical measures such as up-to-date antivirus protection, encryption, and disclosing personal information both internally and to our authorised third party service providers and agents only on a need-to-know basis.

You should be aware, however, that no method of transmission over the Internet or method of electronic storage is completely secure. While security cannot be guaranteed, we strive to protect the security of your information and are constantly reviewing and enhancing our information security measures.

Accuracy Of Your Personal Information

We generally rely on personal information provided by you (or your authorised representative). Choice

You may always choose what personal information (if any) you wish to provide to us. However, if you choose not to provide certain details, some of your experiences with us may be affected (for example, we cannot take a booking without a name).

Whilst we respect your decision to withdraw your consent, please note that depending on the nature and scope of your request, we may not be in a position to continue providing our goods or services to you and we shall, in such circumstances, notify you before completing the processing of your request. Please note that withdrawing consent does not affect our right to continue to collect, use and disclose personal information where such collection, use and disclose without consent is permitted or required under applicable laws.

Access And Updating Your Personal Information

If you wish to make (a) an access request for access to a copy of the personal information which we hold about you or information about the ways in which we use or disclose your personal information, or (b) a correction request to correct or update any of your personal information which we hold about you, you may submit your request in writing or via email to Calibrate at the contact details provided below.

Please note that a reasonable fee may be charged for an access request. If so, we will inform you of the fee before processing your request.

We will respond to your request as soon as reasonably possible. Should we not be able to respond to your request within thirty (30) days after receiving your request, we will inform you in writing within thirty (30) days of the time by which we will be able to respond to your request. If we are unable to provide you with any personal information or to make a correction requested by you, we shall generally inform you of the reasons why we are unable to do so (except where we are not required to do so under the PDPA).

Retention Of Your Personal Information

We may retain your personal information for as long as it is necessary to fulfil the purpose for which it was collected, or as required or permitted by applicable laws.

We will cease to retain your personal information, or remove the means by which the information can be associated with you, as soon as it is reasonable to assume that such retention no longer serves the purpose for which the personal information was collected, and is no longer necessary for legal or business purposes.

Effect Of Policy And Changes To Policy

This Policy applies in conjunction with any other notices, contractual clauses and consent clauses that apply in relation to the collection, use and disclosure of your personal information by us.

We may revise this Policy from time to time without any prior notice. You may determine if any such revision has taken place by referring to the date on which this Policy was last updated. Your continued use of our services constitutes your acknowledgement and acceptance of such changes.
